Chronic use of proton pump inhibitors

  So how careful should we be about chronically using proton pump inhibitors?
  
  I guess that the only cases that would warrant CHRONIC, long term use would be things like megaesophagus and late stage renal disease.
  
  Does it make sense to put all of our end stage renal disease patients on PPI indefinitely?
