This question was submitted in relation to a past webinar titled Canine Addison's Disease ...
Question:
I was recently told by a specialist not to give NaCl as an IV fluid for an Addison's case, because it would increase the sodium levels too quickly. They recommended I use Hartmanns instead. Your thoughts on this, please.
Answer by Elisa Mazzaferro, MS, DVM, PhD, DACVECC:
In animals that have severe hyponatremia (Na+ < 130 meq/L) yes, I would be concerned about raising the sodium too quickly as 0.9% NaCl has 154 mEq/L of sodium and could potentially lead to central pontine myelinolysis if the sodium raises by more than 15 mEq/L in a 24 hour period.
Hartmanns / Lactated Ringers solution has ~ 130 mEq/L of sodium, so is a good fluid for intravascular volume expansion and interstitial dehydration that won't raise the sodium too quickly.
